PRIVACY POLICY

Mending Your Mind Hypnotherapy

Mending Your Mind Hypnotherapy is a Clinical Psychotherapy and Hypnotherapy Practice. I respect your privacy and am committed to protecting the personal information you provide to me.

This Privacy Policy explains what personal information I collect, how and why I collect it, how it is used and stored, who it may be shared with, and your rights under the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

What Information Do I Collect?

I collect personal information when:

  • You complete the contact form on my website.
  • You contact me by email, telephone or text message.
  • You attend hypnotherapy sessions.
  • You complete the CORP Research Programme.
  • You visit my website.

To provide the highest quality support, I may collect:

  • Your name and contact details.
  • Information about what you would like to achieve through      hypnotherapy.
  • Limited relevant health and medical information.
  • Relevant family and medical history.
  • Brief session notes.
  • GP details, with your consent where required.

The CORP Research Programme collects anonymised information to produce statistical analysis relating to Solution Focused Hypnotherapy.

I also collect anonymous information regarding website usage, such as the number of visitors to the site and which pages are accessed.

When you visit the website, you will be asked to accept or decline non-essential cookies.

  

Lawful Basis for Processing

I process personal information in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

Personal information is processed because:

  • Processing is necessary for the performance of a contract to      provide hypnotherapy services (Article 6(1)(b)).
  • Processing is necessary for my legitimate interests in managing and      operating my practice effectively (Article 6(1)(f)).

As part of providing hypnotherapy services, I may process special category data, including limited health information. This information is processed in accordance with Article 9(2)(h) UK GDPR, which permits the processing of special category data for the provision of health or social care.

I only collect information that is relevant and necessary for the safe and effective delivery of therapy.

  

How Is Your Information Stored?

I take appropriate technical and organisational measures to protect your information.

  • Text messages are stored on password-protected devices secured with      fingerprint recognition.
  • Emails are stored on password-protected devices.
  • Electronic documents, including session notes, invoices and      correspondence, are password protected and stored on password-protected      devices.
  • CORP Research information is stored securely on password-protected      devices.
  • Written session notes are stored in a locked cabinet.

  

Retention of Records

As a member of the National Council for Hypnotherapy (NCH), I am required to retain records for eight years following your final session.

For children and young people, records are retained until their 25th birthday. Where treatment ends at age 17, records are retained until the individual's 26th birthday.

Records are securely destroyed during the January following the end of the applicable retention period.

These retention periods are in accordance with professional body requirements and insurance obligations.

  

Your Rights

Under UK GDPR, you have the right to:

  • Request access to the personal information I hold about you.
  • Request correction of inaccurate or incomplete information.
  • Request restriction of processing in certain circumstances.
  • Object to processing where applicable.
  • Request erasure of your personal information where this is legally      possible.
  • Request that your information be transferred to another organisation where appropriate.

Requests will normally be responded to within one month, although this period may be extended where permitted by law.

Please note that I may be required to retain certain records in accordance with professional, legal and insurance obligations.

  

Requests for Access to Your Information

You have the right to request a copy of the personal information I hold about you.

Requests should be made in writing to the address or email shown above. I will normally respond within one month, although this period may be extended where permitted by law.

  

Confidentiality

Everything discussed during hypnotherapy sessions is treated as confidential.

From time to time, I may discuss aspects of my work with my professional supervisor to ensure I provide the highest standard of care. No identifying information about you will be disclosed, and my supervisor is also bound by professional and confidentiality obligations.

  

Contact with Other Health and Social Care Professionals

I will only contact your GP or other Health and Social Care Professionals with your consent, where appropriate.

Exceptions to this include situations where:

  • There is a serious risk of harm to yourself or others.
  • I am legally required to disclose information.
  • Information is required by a court order, warrant or other lawful      authority.

Where possible, I will discuss any necessary disclosure with you before taking action.

  

Meeting Outside Therapy Sessions

I have a duty to protect your confidentiality. Therefore, although I may acknowledge you if we meet outside of therapy, I will avoid initiating conversations regarding your treatment unless you choose to do so.

  

Sharing Information

Your personal information will not be shared with third parties unless:

  • You have provided your consent.
  • Disclosure is required by law.
  • There is a safeguarding concern.
  • Information is required for professional or insurance purposes.
  • Anonymous discussion takes place during professional supervision.

Any information shared will be limited to that which is necessary and appropriate.

I will acknowledge receipt of your complaint within 30 days and will investigate and respond without undue delay. I will notify you of the outcome of the complaint.

If you remain dissatisfied with the outcome, you have the right to raise your concerns with the Information Commissioner's Office (ICO).
